What TogoBridge is
TogoBridge is a secure software portal. It connects the healthcare providers involved in a patient's care.
When you leave the hospital, go home, or start seeing a new provider, your care is handed off. TogoBridge helps your transition-of-care information reach the providers you have connected, at each of those handoffs.
Why it matters to you
Handoffs are where details can slip through the cracks. A new provider may not know what happened at your last visit. You or your family may end up carrying papers, making calls and telling your story again and again.
TogoBridge is meant to take some of that weight off you. When your providers are connected, the information they need can reach them, and you can focus on getting better.
